Registered Nurse - Care Coordination

TMH stands as the premier healthcare institution in the region, featuring a 772-bed acute care facility and offering specialized services including:

  • Level II Trauma Center
  • Level III Neonatal Intensive Care Unit
  • Pediatric Intensive Care Unit
  • Comprehensive cancer, cardiovascular, orthopedic, and surgical programs

Our organization encompasses a psychiatric facility, numerous specialty care clinics, four residency programs, and close to 40 affiliated physician practices.

Key Responsibilities:
  • Execute federally mandated patient evaluations in accordance with Centers for Medicare and Medicaid Services (CMS) standards, ensuring effective care planning and interdisciplinary collaboration.
  • Collect and compile essential data for submission to CMS.
  • Liaise with relevant agencies to facilitate patient transitions to community or long-term care environments.
  • Guarantee adherence to applicable state and federal regulations.
  • Collaborate with a multidisciplinary healthcare team to optimize patient outcomes.

Qualifications:
- Associate degree in nursing.
- Bachelor's degree in nursing preferred.
- Minimum of one (1) year of pertinent experience.
- Two (2) or more years of relevant experience is advantageous.
- Minimal Data Set (MDS) certification or the ability to obtain certification within one (1) year of employment.
