Job Title: Team Nurse

We are seeking a skilled and compassionate Team Nurse to join our Program for Assertive Treatment (PACT) teams. As a key member of our multidisciplinary team, you will provide holistic nursing services to adults with persistent mental illness.

Responsibilities:
  • Conduct psychiatric assessments and physical health evaluations to identify individual needs and develop comprehensive treatment plans.
  • Provide ongoing assessment, problem-solving, and skill training to assist individuals with activities of daily living.
  • Collaborate with the treatment team to coordinate services, monitor progress, and make referrals as needed.
  • Administer medication and provide education on medication management.
  • Develop and maintain relationships with community physicians and other healthcare providers to ensure seamless care coordination.
Requirements:
  • Two years of experience in a mental health setting.
  • RN licensure in the state of service.
  • A high school diploma or equivalent; Bachelor's degree preferred.
  • Strong clinical and crisis intervention skills, with competency in formal and informal assessment tools and practices.
  • Ability to work independently and as part of a multidisciplinary team.
  • Valid US driver's license and a fully insured vehicle for transporting clients.
