Job Summary

We are seeking a highly organized and detail-oriented Specialty Care Coordinator to join our team at JWCH Institute. In this role, you will be responsible for scheduling, contacting patients, and collecting and/or sending required documents for Cardiology, Radiology, Dermatology, Psychiatry, and Podiatry specialty services.

Key Responsibilities
  1. Schedule patients who have been referred for specialty services.
  2. Call patients to remind them about their upcoming appointments.
  3. Collect required pre-assessment paperwork and patient information.
  4. Answer incoming phone calls.
  5. Greet patients and ensure that all paperwork is completed before appointments.
  6. Receive patient medical records and scan them into the Electronic Health Record (EHR).
  7. Provide medical records of patient visits with specialty providers, as needed.
  8. Check patients into their appointments.
  9. Work closely with specialty providers regarding scheduling and follow-up care.
  10. Assist clients in completing forms or providing required information.
  11. Coordinate transportation for patients as needed.
  12. Ensure a clean, orderly, and functional work environment.
  13. Work with IPA on specialty referrals for external services.
  14. Act as a liaison between the primary care provider, patients, specialists, and health plans.
  15. Follow up with patients, reminding them of referral appointments.
  16. Obtain consult notes from external specialists.
  17. Contact the Medical Records Department to retrieve records as needed or requested by the provider.
  18. Manage referral logs and generate reports as needed.
  19. Complete administrative duties as assigned.
Requirements
  1. Experience working in a medical setting.
  2. Excellent oral and written communication skills.
  3. Strong computer skills, including MS Word and Excel.
  4. Experience using Next-Gen Electronic Health Record System or other Electronic Medical Record systems.
  5. Medical Assistant Certification (Preferred).
  6. Experience working with clients and staff from diverse socio-economic, ethnic, and cultural backgrounds.
  7. Bilingual (English and Spanish) preferred.
  8. Ability to work evenings and weekends.
  9. Current California Driver's License.
