Job Summary:

L.A. Care Health Plan is seeking a highly motivated and detail-oriented Care Management Coordinator II to join our team. As a key member of our Care Management department, you will play a critical role in ensuring our members receive high-quality care and support.

Key Responsibilities:

  • Conduct telephonic Health Risk Assessments (HRAs) with high-risk members to identify their strengths, challenges, and unmet needs.
  • Develop and implement care plans in collaboration with the Interdisciplinary Care Team (ICT) to address member needs and improve health outcomes.
  • Coordinate with providers, community-based organizations, and other stakeholders to ensure seamless care transitions and effective resource utilization.
  • Provide education and support to members and caregivers on disease management, health education, and social determinants of health.
  • Maintain accurate and timely documentation of member interactions, care plans, and outcomes.

Requirements:

  • Associate's degree in a healthcare-related field or equivalent experience.
  • At least 6 months of experience in an administrative and customer service role in a healthcare setting.
  • Knowledge of medical terminology and proficiency in Microsoft Office.
  • Excellent communication, organizational, and time management skills.
  • Ability to work effectively in a team environment and demonstrate empathy and compassion for members and caregivers.

Preferred Qualifications:

  • Bilingual in one of L.A. Care Health Plan's threshold languages.
  • Certified Medical Assistant (CMA) certification.
