Care Transition Specialist

Evernorth Health Services, a division of The Cigna Group, is seeking an experienced Registered Nurse to join our team as a Care Transition Specialist. In this role, you will be responsible for coordinating patient care and supporting our clinical team in providing high-quality care to patients transitioning from hospital to home settings.
About the Role
You will work closely with patients, families, and healthcare providers to ensure seamless transitions of care and optimal outcomes. Your strong clinical skills and excellent communication abilities will enable you to provide education and support to patients and their families on disease management, medication adherence, and healthy living strategies.
Key Responsibilities
- Conduct comprehensive assessments to identify patient needs and develop individualized care plans
- Coordinate patient care services, including scheduling appointments, ordering medications, and referring patients to specialists
- Provide education and support to patients and their families on disease management, medication adherence, and healthy living strategies
- Collaborate with healthcare providers to ensure accurate and timely communication of patient information
Requirements
- Active unrestricted compact RN license in all states where Evernorth provides services
- 5+ years of experience as a Registered Nurse, preferably in a telehealth or home healthcare setting
- Strong clinical skills and ability to assess complex patient needs
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations
- Proficiency in Microsoft Office applications and EMR systems, preferably Epic EHR
Salary Information
We offer a competitive salary of $85,000 - $110,000 per year, depending on experience, plus benefits and opportunities for professional growth and development.
