Job Overview

We're seeking a skilled and compassionate Patient Care Specialist to join our team of dedicated professionals. In this rewarding role, you'll provide essential care and support to patients in their homes, working collaboratively with our interdisciplinary team to ensure high-quality care.

About the Job

As a Patient Care Specialist, your responsibilities will include providing personal care to patients, assisting with daily living activities, and reporting changes in their condition to the assigned nurse. You'll also be responsible for adhering to our documentation and care procedures, participating in care conferences, and maintaining a safe and clean environment.

Required Skills and Qualifications
  • Must have certification as a Certified Nursing Assistant (CNA)
  • Valid driver's license, auto liability insurance, and a reliable vehicle
  • Excellent communication and interpersonal skills
  • Ability to lift up to 50 lbs. and move patients

Estimated Salary:
