Job Description

We are seeking a skilled Transitional Care Specialist to join our team at Complete Health. As a key member of our care management team, you will play a vital role in ensuring the smooth transition of patients from hospital to home.

Key Responsibilities
  • Care coordination and communication with healthcare providers, staff, and patients' families
  • Assessment and support for treatment regimen adherence, medication management, and patient education
  • Collaboration with other healthcare professionals to ensure comprehensive care planning
  • Identification of social and preventative care needs and facilitation of resolutions
  • Regular reporting of activities and statistical status of patients in care
Requirements
  • Current Licensed Registered Nurse (RN) in the state of Alabama
  • 3 years of related care management, medical clinical experience, and/or training
  • Athena EHR experience preferred
  • Excellent written and oral communication skills
  • Strong decision-making and documentation skills

Compensation

The estimated annual salary for this position is $72,000-$85,000, depending on experience.
