Job Description

The Community Support Coordinator will be responsible for providing case management services to families in need, with a focus on preventing foster care placement and promoting family stability. The ideal candidate will have a strong understanding of family dynamics, excellent communication and interpersonal skills, and the ability to work independently and as part of a team.

You will be responsible for assessing the needs and strengths of each family, developing and implementing individualized service plans, and providing counseling and support as needed. Additionally, you will maintain accurate records, attend meetings and conferences, and prepare reports as required.

Key Responsibilities:

Requirements:

  • Bachelor's degree in Social Work or related field
  • Experience working with families in a case management or counseling capacity
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Basic computer skills and familiarity with record-keeping software

Salary Range: $70,000 - $90,000 per year, depending on experience.
