As a Clinical Assessment Advisor with CenterWell Home Health, you will play a vital role in delivering high-quality patient care and promoting whole-person health. Our team of dedicated professionals is committed to putting health first, and we are seeking a skilled RN to join our ranks.

Key Responsibilities:
  • Conduct thorough assessments of patient needs and develop personalized care plans to ensure optimal outcomes.
  • Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making.
  • Stay up-to-date on the latest clinical policies, procedures, and regulations to ensure compliance and quality care.
  • Participate in quality improvement initiatives to drive excellence in patient care and outcomes.
Requirements:
  • Bachelor of Science degree in Nursing (BSN) preferred.
  • At least 2 years of clinical home care experience, with a minimum of 1 year in a clinical management role.
  • Current CPR certification.
  • Excellent verbal and written communication skills.
  • Excellent interpersonal skills.
  • Knowledge of state and federal home health agency regulations and compliance standards.
  • Knowledge of clinical policies and procedures and ability to implement.
  • Knowledge of clinical structure of PDGM.
  • Must read, write, and speak fluent English.
  • Must have good and regular attendance.
  • Performs other related duties as assigned.
  • A valid driver's license, auto insurance, and reliable transportation are required.
