Job Summary

CenterWell Home Health is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Registered Nurse. As a key member of our care team, you will provide high-quality patient care and support to individuals in their homes, promoting independence, dignity, and well-being.

Responsibilities
  • Deliver comprehensive and individualized care to patients in their homes, including admission, case management, and follow-up visits.
  • Collaborate with physicians and other healthcare professionals to develop and implement patient care plans, ensuring seamless coordination of care.
  • Conduct thorough assessments, identify patient needs, and develop effective care plans to address those needs.
  • Provide education and support to patients and their families, empowering them to manage their health and make informed decisions.
  • Coordinate with other healthcare professionals, including physical therapists, occupational therapists, and home health aides, to ensure comprehensive care.
  • Document patient care and progress accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Diploma, Associate, or Bachelor's degree in Nursing.
  • Minimum of one year of nursing experience, preferably in home health or a related field.
  • Strong med-surg, ICU, ER, and acute care experience.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Valid driver's license, auto insurance, and reliable transportation.
