Job Summary:

CarePartners is seeking a skilled Physical Therapist to join our Home Health Franklin Team. As a Physical Therapist, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes. Key Responsibilities:

  • Conduct comprehensive assessments and develop individualized plans of care to meet the unique needs of each patient.
  • Provide skilled physical therapy interventions to improve patient mobility, function, and overall quality of life.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
  • Communicate effectively with patients, families, and caregivers to educate and empower them to participate in the care process.
  • Document patient progress and outcomes in a timely and accurate manner.

Requirements:

  • Current North Carolina license to practice Physical Therapy.
  • Driver's license required.
  • BLS/CPR Certification.
  • One to two years of related experience.
  • Preferred experience in acute care settings, particularly in orthopedics, neurology, cardiopulmonary, and wound care.
