About the Role:

CarePartners is seeking a skilled Physical Therapist to join our Home Health Franklin Team. As a Physical Therapist, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes. Your expertise will help patients achieve optimal health outcomes, improve their quality of life, and regain independence.

Responsibilities:

  • Conduct comprehensive assessments and develop individualized plans of care to address patients' physical therapy needs.
  • Provide evidence-based physical therapy interventions to promote patient recovery, mobility, and function.
  • Collaborate with patients, caregivers, and healthcare teams to ensure seamless care coordination and communication.
  • Monitor patient progress, adjust treatment plans as needed, and document outcomes to ensure high-quality care.
  • Stay up-to-date with industry developments, best practices, and regulatory requirements to maintain expertise and provide exceptional care.

Requirements:

  • Current North Carolina license to practice Physical Therapy.
  • Driver's license required.
  • BLS/CPR Certification.
  • One to two years of related experience.
  • Preferred experience in acute care settings, especially orthopedics, neurology, cardiopulmonary, and wound care.
