About the Role

We are seeking a skilled Physical Therapist to join our Home Health team at CarePartners. As a Physical Therapist, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes.

Key Responsibilities
  • Manage an assigned caseload of patients, providing therapy and clinical case management to improve patient outcomes and manage resources effectively.
  • Conduct comprehensive assessments, develop and modify plans of care in conjunction with patients, caregivers, and physicians, and provide treatment visits to facilitate patient engagement and achievement of goals.
  • Communicate handoff information and coordinate with Physical Therapist Assistants, other therapists, and members of the care team to support patient care throughout the home health continuum.
Requirements
  • Current North Carolina license to practice Physical Therapy.
  • Driver's license required.
  • BLS/CPR Certification.
  • One to two years of related experience.
  • Basic Cardiac Life Support must be obtained within 30 days of employment start date.
  • Licensed Physical Therapy must be obtained within 2 years of employment start date.
