About the Role

CarePartners is seeking a skilled Physical Therapist to join our Home Health team in Franklin. As a Physical Therapist, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes. Your expertise will help patients achieve their goals, improve their functional abilities, and enhance their overall quality of life.

Responsibilities

  • Conduct comprehensive assessments and develop individualized plans of care to address patients' physical therapy needs.
  • Provide skilled physical therapy interventions, including exercises, modalities, and education, to promote patient progress and independence.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and effective communication.
  • Document patient progress, outcomes, and care plans in accordance with regulatory requirements and CarePartners' policies.
  • Participate in ongoing education and professional development to stay current with best practices and evidence-based care.

Requirements

  • Current North Carolina license to practice Physical Therapy.
  • Basic Cardiac Life Support certification.
  • One to two years of experience in home health or a related field.
  • Preferred experience in acute care settings, particularly in orthopedics, neurology, cardiopulmonary, and wound care.
