About the Role

Pinnacle Home Care Inc. is seeking a skilled and compassionate PRN Speech Therapist to join our team of healthcare professionals. As a PRN Speech Therapist, you will play a vital role in providing high-quality speech therapy services to our patients in their homes.

Key Responsibilities
  • Evaluate patients' level of functioning and hearing, and recommend mechanisms to enhance their hearing ability.
  • Provide initial and ongoing comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
  • Establish a speech therapy treatment plan in consultation with the physician and revise it with the physician's approval as necessary.
  • Assist in the development and implementation of the interdisciplinary client care plan as it pertains to speech therapy.
  • Communicate with other team members and instruct them in speech pathology techniques as needed.
  • Teach, supervise, and counsel patients and their families in the total speech therapy program and other related problems.
  • Record all evaluation data, treatments, and patients' responses to therapeutic intervention in a timely manner.
  • Supervise Home Health Aides as needed and record and report patients' reactions to treatment and any changes in their condition to the physician.
  • Participate in the development of periodic revisions to the physician's plan of treatment and communicate with the physician at least every 30 days or whenever changes occur.
  • Evaluate the effectiveness of speech pathology intervention and update the client care plan as needed.
Requirements
  • Graduate of an accredited speech therapy program or equivalent education.
  • Licensed in the State of Florida as a Speech Therapist.
  • Two years of clinical experience as a Speech Therapist.
  • Current BLS Certification.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
  • Ability to physically tolerate much standing, stooping, and bending, and meet all agency regulations.
  • Self-directed and able to work with little supervision, with good observational, judgment, and communication skills.
