Key Responsibilities:
* Provide expert care and support to patients in the comfort of their own homes
* Collaborate with the healthcare team to coordinate care and ensure seamless transitions between settings
* Develop and implement personalized care plans that address the unique needs and goals of each patient
* Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal outcomes

Requirements:
* 2+ years of experience in healthcare or a related field
* Associate's degree or higher in a healthcare-related field; bachelor's degree preferred
* Strong knowledge of healthcare principles and practices
* Excellent communication and interpersonal skills
* Ability to lift up to 50 pounds and stand for long periods of time

Salary:
$55,000 - $65,000 per year, depending on experience and qualifications.
