Position Overview

We are looking for a compassionate and detail-oriented individual to fulfill the role of Qualified Medication Administration Personnel (QMAP). The QMAP plays a crucial role in the safe and precise delivery of medications to residents, under the supervision of a licensed nurse. The ideal candidate will possess strong attention to detail, exceptional communication abilities, and a genuine passion for delivering high-quality care.

Available Shift:

  • Part Time/PRN - Monday 7:00am - 11:00am

Key Responsibilities

Assist in fostering a physical, social, and psychological environment that promotes the best interests and welfare of residents.

Administer or assist with self-administration of medications and treatments as directed by healthcare providers, while observing and reporting responses to existing care plans in accordance with state regulations and required training/certification.

Maintain the cleanliness of the medication supply area, conduct audits of medication carts, and ensure accurate tracking of all medications.

    • Participate in required in-service training, educational classes, and meetings.
    • Read and comprehend instructions, correspondence, and memos.
    • Collaborate effectively within an interdisciplinary team.
    • Maintain a comprehensive understanding of Community Electronic Health Record software.
    • Adhere to Community policies and procedures.
    • Perform basic mathematical calculations.
    • Make independent decisions when necessary.
    • Uphold the highest standards of personal and professional integrity.
    • Respect and honor Resident Rights.
    • Communicate with residents in a dignified manner.
    • Respond promptly and appropriately to requests from residents' families or visitors.
    • Provide courteous and effective service to all residents, guests, and colleagues.
  • Medication Administration Duties
    • Ensure that the medication room and cart are secured at all times.
    • Keep the medication cart organized, clean, and well-stocked.
    • Order, store, and administer medications in accordance with Community policies and physician orders.
    • Conduct narcotic counts during each shift and report any discrepancies to the Health Wellness Director.
    • Review daily notes for updates and changes at the start of each shift.
    • Dispose of expired or discontinued medications promptly according to Community policies.
    • Obtain and document vital signs as directed on the Medication Administration Record (MAR) or vital sign log in the resident's electronic health record.
    • Follow Universal Precautions and Infection Control procedures.
    • Maintain logs for medication and specimen refrigerator temperatures.
  • Observation, Documentation, and Reporting
    • Regularly review resident care plans and medication orders to stay informed of any updates or changes.
    • Complete Incident Reports in accordance with Community Policy and Procedure.
    • Document Short Term Observation in the resident's Electronic Health Record (EHR) as applicable.
    • Record the support and assistance provided in the resident's EHR when necessary.
    • Document and report any requests for additional support or changes in assistance in the resident's EHR when applicable.
    • Document and report observations of any unusual or significant changes in residents' physical, mental, or behavioral conditions in the resident's EHR when applicable.
  • Resident Care Responsibilities
    • May provide direct care to residents as needed.
    • When delivering resident care:
      • Act with respect for dignity, personal preferences, and choices.
      • Encourage resident independence in line with care plans.
      • Adhere to Community infection control guidelines.
  • Physical Demands and Working Conditions
    • Ability to sit, stand, bend, lift, push, pull, stoop, walk, and move frequently during working hours.
    • Capable of lifting 40 lbs. from floor to waist, lifting 10 lbs. from waist to shoulder, carrying 40 lbs., and pushing/pulling 40 lbs.
    • Able to assist in the evacuation of residents when necessary.
  • Qualifications and Educational Requirements
    • Must effectively communicate in English and follow written and verbal instructions.
    • High School Diploma or equivalent is required.
    • Qualified Medication Administration Person (QMAP) certification as mandated by Colorado regulations.
    • Current CPR certification is required throughout employment.
    • First aid certification from a recognized organization is preferred.
    • A minimum of one year of experience in a senior care or healthcare setting is preferred.
