Key Responsibilities:

- Assess physician medication orders for suitability regarding drug selection, dosage, potential interactions, administration routes, and stability. Propose therapy modifications to physicians and facility personnel as necessary.
- Conduct monthly reviews of the medication regimens and clinical indicators for each assigned resident, reporting any discrepancies to the Director of Nursing, Administrator, and attending physicians, accompanied by a written summary of findings and suggestions.
- Maintain comprehensive documentation of all interactions with physicians, nursing staff, other healthcare team members, patients, and their families.
- Stay informed about pharmacy laws and federal and state regulations related to long-term care.
- Collaborate effectively as part of a healthcare team.
- Keep updated on pharmaceutical care practices relevant to long-term care residents.

Qualifications:
- Bachelor's degree from an accredited School of Pharmacy or a Doctor of Pharmacy degree.
- A current pharmacy license is required; dual licensure is preferred.
- A minimum of one year of experience in acute care, long-term care, or home health is preferred.
- Experience as a Consultant or Clinical Pharmacist is advantageous.
- IV clinical experience is a plus.
- Flexibility to work weekends or outside standard business hours when necessary.
- Capability to lift patient charts repeatedly.
- Ability to stand for extended periods.
- Strong customer service skills.
- Capacity to learn and comprehend state and federal regulations governing long-term care.
- Understanding of formulary management.
