Summary:
Under the supervision of the Director of Pharmacy, the Clinical Pharmacist is accountable for ensuring the safe and effective use of medications throughout the healthcare facility.
This role involves primary responsibilities such as interpreting medication orders, preparing and dispensing pharmaceuticals, and providing comprehensive drug information.Education:
PharmD degree or a Bachelor's degree in Pharmacy from an accredited institution.
Certification, Registration & Licensure:
Licensure required:
NH Pharmacist license or pending reciprocity.
Board Certified Pharmacotherapy Specialist (BCPS) is preferred.
Experience:
A Pharmacy residency is advantageous.
Key Responsibilities:
1. Identify and resolve actual or potential medication-related issues.
2. Take ownership of pharmacy-related matters and ensure their resolution.
3. Provide accurate drug information to healthcare staff, providers, and the community.
4. Assist physicians and residents in developing optimal drug therapies for patients, including pharmacokinetic dosing and monitoring services.
5. Establish a pharmacy practice specialty or area of expertise, serving as a resource for this specialty by providing education, monitoring, and support to enhance departmental quality.
6. Document and monitor all interventions and their outcomes.
7. Perform medication order entry in accordance with current departmental policies and procedures.
8. Coordinate the daily distribution process of the pharmacy.
9. Represent the pharmacy in hospital-wide committees and participate in pharmacy committees.
10. Provide written and/or in-service education regularly.
11. Undertake additional projects or duties as assigned.
