Nurse Site Lead

The Site Lead Registered Nurse is a key member of our healthcare team, responsible for guiding and supervising assigned nursing personnel in delivering high-quality patient care. This role requires strong leadership skills, clinical expertise, and the ability to collaborate with nurses, physicians, and other healthcare professionals.
Key Responsibilities:- Perform comprehensive patient assessments, data collection, and modify patient care plans as needed.
- Collaborate with nurses, physicians, and other healthcare professionals to deliver patient care.
- Assume responsibility for managing a selected caseload of patients, prioritizing care based on urgency and department standards.
- Perform in-person and phone triage, ensuring timely and accurate documentation of patient information.
- Develop and implement patient-specific goal-directed plans of care, prioritizing patient outcomes and seeking participation from patients and significant others.
- Ensure total patient care needs are met by following established multi-disciplinary plans of care.
- Implement care in a knowledgeable, skillful, consistent, and holistic manner, including appropriate patient follow-up.
- Utilize knowledge of dosage calculations, indications, actions, routes, side effects, and patients' rights for proper administration of medications.
- Assist in recruitment, orientation, training, and retention of new staff, including LPNs and MAs.
- Work with Providers to assess performance of LPNs and MAs.
- Graduate from an accredited school of nursing.
- Current Registered Nurse licensure in Connecticut.
- CPR certification.
- Minimum of one year experience in a long-term care facility or rehabilitation center.
- Strong motivation and ability to accept supervision.
- Computer skills, including ability to learn EMR and EPIC medical records.
- Ability to provide phone triage and knowledge of referral process, medication refill procedures, and administration.
- Knowledge of CLIA waived lab testing, Urine Dipsticks, and Glucometers.
- Excellent leadership qualities and knowledge of management skills.
- Bilingual individual is desirable but not required.
