Key Responsibilities
  • Leadership and Supervision: Provide guidance and supervision to assigned nursing personnel, ensuring the delivery of high-quality patient care.
  • Patient Care Management: Manage a selected caseload of patients, prioritizing their needs and developing individualized care plans.
  • Collaboration and Communication: Work closely with nurses, physicians, and other healthcare professionals to ensure seamless patient care.
  • Documentation and Reporting: Maintain accurate and timely documentation of patient care, including assessments, interventions, and outcomes.
  • Training and Development: Assist in the recruitment, training, and retention of new staff, ensuring they have the necessary skills and knowledge to provide high-quality patient care.
  • Performance Evaluation: Work with Providers to assess the performance of LPNs and MAs, identifying areas for improvement and developing strategies for growth.
Requirements
  • Education and Licensure: Graduate from an accredited school of nursing and hold a current Registered Nurse license in the state of Connecticut.
  • Experience: Minimum of one year of experience in a long-term care facility or rehabilitation center, with clinic experience highly desirable.
  • Skills and Qualities: Excellent patient management skills, strong leadership qualities, and the ability to work effectively in a fast-paced environment.
  • Computer Skills: Proficient in electronic medical records (EMR) and EPIC medical records, with the ability to learn new systems quickly.
