Job Summary:
Great Lakes Integrated Network, Inc. is seeking a highly motivated and detail-oriented Pharmacy Professional to join our team. As a Pharmacy Professional, you will play a vital role in assisting the pharmacy department with quality reporting, closing medication-related gaps in care, and other day-to-day tasks assigned by management.
Key Responsibilities:
- Quality Reporting and Improvement: Assist with the management of quality incentive projects to improve GLIN IPA's performance in payer contracting. Under the supervision of a pharmacist, roles and responsibilities include:
- Provide adherence counseling to patients
- Perform chart reviews relative to medication-related HEDIS/STAR measures
- Recommend therapy adjustments to providers to optimize medication regimens
- Counsel patients on medication changes and provide solutions for related issues
- Medication Therapy Reviews: Complete medication therapy reviews, both for comprehensive and targeted intervention opportunities
- Drug Information and Education: Develop drug information newsletters to communicate relevant practice updates to network providers
- Data Tracking and Analysis: Assist with data tracking team performance
- Other Duties: Perform other duties assigned by management
Requirements:
- Education: Must have completed at least one year of pharmacy schooling and have a current, active NYS Pharmacist Intern license
- Experience: ≥1 year of previous community pharmacy-related work experience preferred, but not required
- Skills: Ability to utilize clinical practice guidelines to make clinical recommendations and counsel patients on medication regimens; foundational understanding of HEDIS and STAR quality measures helpful for this position; strong written and verbal interpersonal communication skills; ability to work effectively as part of a multidisciplinary team; proficiency in Microsoft Excel, preferred; ability to work independently, as well as multitask
What We Offer:
Great Lakes Integrated Network, Inc. offers an outstanding internship experience and a great working environment. This position is conducted both onsite and remotely, when applicable. Working requirements are flexible, up to 40 hours per week during school breaks, and between 8 & 10 hours per week during the semester.
Compensation: Pay range $15.00-$22.00 per hour based on experience and education levels
