Community Support Liaison

We are seeking a Community Support Liaison to join our interdisciplinary team. As a key member of the team, you will work closely with nurses and care managers to support older adults and individuals with disabilities in their homes.
The successful candidate will promote health and well-being through education and coordination of services. Key responsibilities include conducting assessments, promoting member involvement, and assisting with care transitions, follow-up, and accessing resources.
This is a full-time position (35 hours/week) with a competitive salary range of $52,200 - $63,000 per year, depending on experience. Excellent benefits include generous PTO, medical/dental/vision insurance, Flex spending, EAP, 401k, and more.
Requirements:
- High school diploma with 1 year of relevant experience required. Bachelor's Degree preferred.
- Reliable transportation and ability to travel throughout Franklin, Hampshire, Hampden, and Northern Worcester Counties.
- Demonstrated experience working with older adults or individuals with disabilities, including experience navigating benefits, resources, behavioral health systems, and long-term supports within the community.
