Home Health Clinical Lead

We are seeking a highly motivated Home Health Clinical Lead to join our team. As a key member of our home health team, you will be responsible for leading a team of clinicians to deliver high-quality patient care and improving patient outcomes.
Key Responsibilities:
- Clinical Team Leadership: Provide strong leadership and direction to a team of clinicians to ensure that they have the necessary resources and support to deliver exceptional patient care.
- Clinical Practice Development: Collaborate with clinicians to develop and implement evidence-based clinical practices that improve patient care.
- Care Management Coordination: Coordinate the provision of nursing care to patients in their homes as prescribed by physicians and in compliance with applicable laws and regulations.
Requirements:
- Education and Experience: Current RN licensure in the state of practice; current CPR certification required.
- Leadership Skills: Strong leadership skills with the ability to motivate and inspire a team to deliver exceptional patient care.
- Clinical Expertise: Demonstrated expertise in home health nursing practice and ability to lead a team of clinicians to deliver high-quality patient care.
Salary Range: $110,000 - $140,000 per year.
