Salary
The starting salary for this position is $104,000+ per year, depending on experience.
Job Description
- We are seeking an experienced RN with Home Health experience to oversee the supervision and coordination of clinical services.
- As a Patient Care Manager, you will provide and direct provisions of nursing care to patients in their homes as prescribed by the physician.
- You will ensure patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
Required Skills and Qualifications
- Current RN licensure in state of practice
- Current CPR certification required
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
