Established in 1976, the Center provides comprehensive case management and integrated social services without discrimination. Annually, it supports approximately 6,000 individuals through its extensive programs.
Role Summary
The Case Manager is an integral member of the Integrated Mobile Health Team – Full Service Partnership (IMHT-FSP). This role involves proactive outreach to chronically homeless individuals in the community, delivering intensive services aimed at engaging them in the service continuum and facilitating their transition to housing readiness. The Case Manager will assist clients in applying for subsidized housing and connecting them with available housing resources, ensuring a smooth transition into permanent housing. Post-housing, the Case Manager will provide ongoing support to help clients maintain their housing and integrate into the community.
Key Responsibilities
- Establish collaborative relationships with outreach workers, law enforcement, medical personnel, and housing authorities.
- Identify and prioritize outreach efforts towards the most vulnerable chronically homeless individuals.
- Collaborate with other agencies to develop actionable service plans that promote housing opportunities and stability.
- Assist in creating and implementing individualized treatment plans addressing mental health and substance abuse challenges.
Essential Duties
- Conduct regular assessments of client progress towards their goals.
- Utilize evidence-based practices, including Harm Reduction and Motivational Interviewing, in client interactions.
- Deliver comprehensive case management services to ensure successful housing retention and community integration.
- Support clients in navigating housing applications, benefits, and essential life skills.
- Provide crisis intervention and therapeutic services in various settings.
- Engage in advocacy efforts to prevent evictions and connect clients with necessary services.
- Maintain accurate documentation in compliance with agency and program requirements.
Qualifications
- Strong interpersonal skills and the ability to engage with diverse populations.
- In-depth knowledge of homeless services and best practices in case management.
- Flexibility to work varied hours, including early mornings and evenings.
Experience and Education
- Experience in providing services to homeless individuals is preferred.
- A Bachelor's degree in social work, human services, or a related field is required.
Additional Information
This position does not have supervisory responsibilities. Proficiency in MS Office and a valid California Driver License are required. Travel to meet clients and attend meetings is necessary.
