Job Summary

The RN Care Coordinator plays a vital role in planning, coordinating, and managing the care provided to assigned patients at Centra Health. This position requires a professional registered nurse who can lead the interdisciplinary team in creating a comprehensive plan of care.

The ideal candidate will have one year of external nursing experience or completion of a Nurse Residency Program. A current, active license as a registered nurse in Virginia or a multistate licensure privilege is required. Additionally, the candidate must possess a valid American Heart Association Basic Life Support (AHA BLS Provider or AHA BLS Instructor) course completion card.

Key Responsibilities
  • Plans, coordinates, and manages the care provided to assigned patients
  • Delegates to care providers according to the ANA Principles of Nurse Staffing
  • Thinks critically and makes appropriate clinical decisions
  • Leads and communicates with the interdisciplinary team to plan patient care
  • Creates and maintains a safe patient environment
  • Participates in initiatives resulting from nurse-sensitive quality indicators and/or unit-based quality metrics
Qualifications
  • Graduate of an accredited Nursing Program
  • One year of external nursing experience or completion of Nurse Residency Program
  • Current, active license as a registered nurse in Virginia or multistate licensure privilege
  • American Heart Association Basic Life Support (AHA BLS Provider or AHA BLS Instructor) course completion card
