At UnitedHealth Group, we're committed to simplifying the healthcare experience and creating healthier communities. As a RN Field Care Coordinator, you'll play a vital role in coordinating the long-term care needs for patients in the local community.

You'll work closely with an elite and dynamic team to assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care.

Key responsibilities include:

  • Assessing patient needs and developing personalized care plans
  • Identifying and initiating referrals for social service programs
  • Managing the care plan throughout the continuum of care as a single point of contact
  • Communicating with stakeholders to ensure quality coordinated care and services are provided expeditiously to all members

Required qualifications include:

  • Current, unrestricted RN license in TN
  • 5+ years of experience working with MS Word, Excel, and Outlook
  • 4+ years of experience working within the community health setting in a healthcare role
  • Experience in long-term care, home health, hospice, public health, or assisted living

Preferred qualifications include:

  • Certified Case Manager (CCM)
  • Experience with electronic charting
  • Case management experience
  • Experience or exposure to discharge planning
  • Experience in utilization review, concurrent review, or risk management
  • Experience in a telephonic role
  • Experience with arranging community resources
  • Background in managing populations with complex medical or behavioral needs
