Core Responsibilities

The Care Manager plays a pivotal role in orchestrating patient care to ensure safe, efficient, and timely transitions throughout the healthcare continuum.

This position employs a collaborative approach, wherein the care manager identifies, assesses, plans, implements, and evaluates the necessary options and services to address an individual's health and related needs, taking into account social determinants that influence overall well-being.

The care manager is tasked with the screening, identification, and assessment of individuals requiring active Case Management services, ensuring that appropriate resources are allocated at the right time and place.

Key Duties
  • Identifies and prioritizes patients needing care management services through a holistic lens, considering biopsychosocial, functional, cultural, spiritual, and financial factors.
  • Collaborates with patients, families, caregivers, and healthcare team members to optimize healthcare responses, quality, and cost-effective outcomes, while monitoring and adjusting plans as necessary.
  • Maintains comprehensive and timely documentation in patient records, reflecting physical and functional limitations, psychosocial characteristics, and educational needs of patients and their families.
  • Initiates referrals to relevant disciplines as required, ensuring that care plans address post-hospital needs and involve patient and family participation.
  • Follows established practices related to Advance Care Planning, length of stay management, readmission prevention, and transitional care management.
  • Facilitates proactive care for inpatients and observation patients, addressing barriers to timely testing and treatment, and ensuring appropriate resource utilization.
  • Collaborates with revenue cycle partners to eliminate barriers, ensuring patients are classified correctly as guided by physicians.
  • Engages in clinical outcome projects and process improvement initiatives within care management.
  • Fosters strong collaborative relationships with nursing staff, physicians, and other care team members to achieve departmental and facility goals.
  • Adheres to compliance regulations and identifies situations requiring referral to other healthcare team members.
Qualifications

Licensing/Certification:
BLS Basic Life Support – American Heart Association (required)
Current RN License in the state of practice or covered by compact.

Preferred:
Case Management Certification (ACM, CCM, or ANCC) preferred upon hire, required within three years of hire.

Education:
Associate degree in Nursing (required for RNs; must enroll in BSN program within two years of hire and complete within five years).

Experience:
Minimum of one year in a clinical setting (required)
1-2 years in an acute care clinical environment (preferred)
Experience in ambulatory or post-acute care coordination (preferred).
