Job Title: Case Manager Supervisor

Pinnacle Services is a dynamic and innovative agency seeking a skilled Case Manager Supervisor to lead our team of Case Managers and manage our health plans and County Contracts. If you have experience working with the various aging and disability waivers and the Minnesota Health Care Programs, this could be an excellent opportunity for you.

Job Responsibilities:

  • Attend County meetings to stay up-to-date on policy and procedural changes.
  • Assist in the training of new Case Managers and supervisors to ensure a smooth onboarding process.
  • Provide support to Case Manager Captains (supervisors) as their immediate supervisor, helping them to achieve their goals and objectives.
  • Ensure completion of department goals and objectives, including contract and licensing compliance.
  • Contribute to business development by identifying new opportunities and implementing strategies to achieve growth.
  • Remain up-to-date on policy and procedural changes to ensure compliance and best practices.

Requirements:

  • Graduate from an accredited four-year college with a major in social work, psychology, sociology, or a closely related field.
  • Graduate from an accredited four-year college with a major in any field and one year of experience as a social worker/case manager/care coordinator in a public or private social service agency.
  • Three to five years of supervisory experience in social services, preferably in case management services.
  • Two or more years of experience with waivered Case Management (CADI, DD, AC/EW).
  • Valid driver's license and acceptable driving record, as well as a vehicle covered with current insurance.
  • Ability to work from our office with remote flexibility one day per week after 90 days.
