Case Manager

Avenues for Youth is seeking a dedicated and compassionate Case Manager to join our team at Minneapolis Avenues. As a key member of our Shelter and Transitional Living department, you will play a vital role in providing case management services to youth experiencing homelessness.
Key Responsibilities- Conduct comprehensive assessments of youth to identify their needs and develop individualized goal plans.
- Provide mentoring and guidance to help youth achieve their goals, including financial support, education, employment, and housing.
- Coach youth on independent living skills, such as establishing routines and preparing for stable housing, school, work, and relationships.
- Collaborate with colleagues and community partners to coordinate care and provide proactive intervention in conflict situations.
- Enter data into Homeless Management Information Systems (HMIS) and complete outcomes reporting.
- Bachelor's degree in social work, human services, education, or a related field.
- Minimum of three years of related experience and/or training, or an equivalent combination of education and experience.
- Previous experience with trauma-informed care, strengths-based youth empowerment models, HMIS, VI-SPDAT, and IAPP is preferred.
Avenues for Youth is committed to providing a supportive and inclusive work environment.
