Job Overview

Position Summary:

The Clinical Care Coordinator (RN) plays a pivotal role in collaborating with healthcare providers and interdisciplinary teams to formulate a comprehensive care strategy for each patient from the point of admission until discharge. This role is essential in ensuring that patients are making progress towards their health objectives by consistently evaluating their care through ongoing assessments and feedback. The Coordinator identifies and addresses patient and family needs by orchestrating the efforts of various team members. They also pinpoint and resolve obstacles that may impede effective patient care. The Coordinator is responsible for strategically planning to meet patient requirements, managing the duration of stay, and optimizing resource utilization, which includes facilitating patient care throughout the healthcare continuum and intervening as necessary to eliminate barriers to timely and effective care delivery.

Key Responsibilities:

  • Develops and oversees the patient's care plan to ensure the effectiveness and appropriateness of services provided.
  • Recognizes and escalates systemic barriers that hinder diagnostic or treatment advancements.
  • Actively identifies and resolves delays and challenges related to patient discharge.
  • Utilizes advanced conflict resolution skills to ensure prompt resolution of issues.
  • Collaborates with medical, nursing, and ancillary staff to eliminate barriers to efficient care delivery in the suitable environment.
  • Ensures that all critical elements of the care plan are communicated to the patient/family and healthcare team members, with necessary documentation to guarantee continuity of care.
  • Manages all aspects of discharge planning for assigned patients.
  • Provides interventions that reflect knowledge and sensitivity to cultural diversity, as well as the religious, developmental, health literacy, and educational backgrounds of the patient population.
  • Evaluates the patient's formal and informal support systems, along with available benefits and community resources.
  • Engages directly with patients and families to assess needs and create an individualized care plan in partnership with the physician.
  • Ensures consensus on the care plan among the patient/family, physician, and payor.
  • Offers education, information, guidance, and support related to the patient's care objectives.
  • Acts as a patient advocate to formulate treatment plans and coordinate care, facilitating transitions to the appropriate next level of care.
  • Upholds and promotes respect for the dignity and rights of every patient while adhering to safety standards and practices within the organization and nursing profession.
  • Collaborates with physicians and other healthcare professionals to encourage the appropriate use of medical center resources.
  • Provides information and support to patients and families, assisting them in accessing necessary resources within the medical center and community.
  • Actively participates in clinical performance improvement initiatives related to length of stay, resource utilization, avoidable days, cost per case, and readmissions.
  • Evaluates the effectiveness of interventions through direct communication with post-acute care providers, patients, and caregivers.
  • Promotes individual professional growth and development by fulfilling requirements for mandatory/continuing education and skills competency.

Qualifications:

  • Bachelor of Science in Nursing (BSN) from an accredited nursing program or demonstrated success in a similar nursing role.
  • Exceptional verbal and written communication skills.
  • Strong critical and analytical thinking abilities.
  • Proven clinical competency.
  • Familiarity with discharge planning, utilization management, case management, performance improvement, and managed care reimbursement.
  • Understanding of pre-acute and post-acute care levels and community resources.
  • Ability to work independently and exercise sound judgment in interactions with physicians, payors, patients, and their families.
  • A minimum of three years of clinical experience in a healthcare setting or demonstrated success in a similar nursing role.
  • Experience in case management and utilization review is preferred.
  • Current RN License in the state of employment or compact required.
  • Certification in Case Management or demonstrated success in a similar nursing role.
