Job Summary

We are seeking a highly skilled Patient Care Management Coordinator to join our team at Fairview Health Services. This is a critical role that requires a strong background in nursing and experience in care coordination.

Key Responsibilities
  • Comprehensive Care Planning
    • Develop and implement individualized care plans for patients across the healthcare continuum.
    • Assess patient needs and develop strategies to achieve optimal outcomes.
  • Care Coordination
    • Serve as a central point of contact for patients, physicians, and care providers.
    • Initiate and oversee transition activities for patients, ensuring seamless care transitions.
  • Patient Education
    • Assess and address patient and family learning needs, developing customized education plans.
    • Provide education and support to patients and families to promote self-management and independence.
  • Clinical Collaboration
    • Collaborate with the healthcare team and patients' families to manage discharge plans effectively.
    • Communicate care plans effectively across the continuum of care.
  • Quality Improvement
    • Participate in process improvement initiatives to enhance clinical efficiency and satisfaction.
    • Identify opportunities to streamline care processes, reduce delays, and ensure safe discharges.
  • Regulatory Compliance
    • Collect necessary data for utilization and regulatory compliance.
    • Communicate any case escalation issues promptly to the appropriate stakeholders.
  • Leadership and Mentorship
    • Establish strong relationships with physicians, nurses, payers, and other stakeholders.
    • Act as a critical link in communication between team members, physicians, and payers.
    • Mentor other healthcare team members on case management best practices.
    Requirements
    • AD in Nursing
    • 5 years of clinical experience
    • Minimum of 1 year working as a care coordinator or case manager
    • Active MN Registered Nurse license

Preferred Qualifications

  • Case Management Certification
  • BSN
  • Basic Life Support (BLS) certification

Additional Requirements

  • Understanding of hospital and community resources, as well as resource/utilization management
  • Proficiency in evidence-based guidelines
  • Strong critical thinking, problem-solving, communication, and time management skills
  • Ability to work effectively in an interdisciplinary team
  • Familiarity with computer systems and Microsoft applications
