We are seeking a highly skilled and experienced Clinical Assessment Advisor to join our team at CenterWell Primary Care. As a key member of our home health care team, you will play a critical role in ensuring the highest level of care for our patients.

Key Responsibilities
  • Develop and Implement Care Plans: Work with patients, families, and healthcare teams to develop and implement comprehensive care plans that meet the unique needs of each individual.
  • Conduct Clinical Assessments: Conduct thorough clinical assessments to identify patient needs and develop effective care plans.
  • Coordinate Care Teams: Coordinate with healthcare teams, including nurses, therapists, and home health aides, to ensure seamless care delivery.
  • Monitor Patient Progress: Regularly monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
  • Communicate Effectively: Communicate effectively with patients, families, and healthcare teams to ensure clear understanding of care plans and progress.
Requirements
  • Education: Bachelor of Science degree in Nursing (BSN) preferred.
  • Experience: At least 2 years of clinical home care experience with a minimum of 1 year in a clinical management role.
  • Certifications: Current CPR certification.
  • Skills: Excellent verbal and written communication skills, excellent interpersonal skills, knowledge of state and federal home health agency regulations and compliance standards, and ability to implement clinical policies and procedures.
