About the Role

We are seeking a skilled Clinical Assessment Advisor to join our team at CenterWell Home Health. As a key member of our care team, you will be responsible for conducting thorough assessments of patients' needs and developing personalized care plans to ensure optimal clinical outcomes.

Key Responsibilities
  • Conduct initial and ongoing evaluations of patient needs, identifying areas for improvement and developing strategies for care.
  • Collaborate with healthcare professionals, including physicians and referral sources, to obtain necessary clinical information and ensure seamless care coordination.
  • Develop and implement care plans that incorporate teaching related to health maintenance, prevention, and safety.
  • Coordinate available resources to manage care plans and ensure stated outcomes are achieved.
  • Periodically reassess patient needs and revise care plans as necessary.
  • Promote effective communication between team members, attending physicians, administrative staff, referral sources, and external case managers to ensure appropriateness of care.
  • Ensure patient needs are identified and qualified caregivers are assigned to all cases within their scope of practice.
Requirements
  • Bachelor of Science degree in Nursing (BSN) preferred.
  • At least 2 years of clinical home care experience, with a minimum of 1 year in a clinical management role.
  • Current CPR certification.
  • Excellent verbal and written communication skills.
  • Excellent interpersonal skills.
  • Knowledge of state and federal home health agency regulations and compliance standards.
  • Knowledge of clinical policies and procedures and ability to implement.
  • Knowledge of clinical structure of PDGM.
  • Must read, write, and speak fluent English.
  • Must have good and regular attendance.
  • Approximate percent of time required to travel: 60%.
