Overview:

AccentCare is seeking a skilled Licensed Vocational Nurse Clinical Support Specialist to join our team. As a professional in the field of corporate healthcare, you will have the opportunity to reimagine care and provide world-class patient service.

Key Responsibilities:

  • Provide clinical support to patients and families in a home care setting
  • Collaborate with healthcare teams to develop and implement patient care plans
  • Utilize Electronic Medical Records (EMR) to document patient information and track progress
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care delivery
  • Participate in ongoing education and training to stay current on industry developments and best practices

Requirements:

  • RN or LVN/LPN with current licensure to practice nursing in the practicing state
  • Possess and maintain CPR certification
  • Previous experience in home care setting preferred
  • Prior use of Electronic Medical Records (EMR) preferred
  • Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
  • Demonstrates excellent observation, nursing judgement, and communication skills
  • Must be a licensed driver with an automobile in good working order
