Key Responsibilities:

  • Conducting thorough assessments of patients' health conditions and nursing requirements.
  • Establishing health objectives tailored to each patient.
  • Monitoring and documenting patients' symptoms closely.
  • Communicating any adverse reactions to treatments and changes in patients' physical and emotional states to the case manager.
  • Ensuring continuity of care by collaborating effectively with nursing staff and other healthcare professionals to uphold the standards of nursing practice in home settings.
  • Providing education, guidance, and support to patients and their families regarding the care plan.

You will report directly to the RN Team Leader and the Director of Nursing.

A valid RN license is mandatory. While prior home health experience is advantageous, it is not a prerequisite.

We value a positive attitude, exceptional nursing abilities, strong communication skills, and a proactive approach.

This position offers flexible scheduling, allowing you to manage your time effectively. Compensation is competitive and commensurate with experience.
