Your responsibilities will encompass:
- Evaluating patient conditions and nursing requirements
- Establishing health objectives for patients
- Monitoring patient symptoms closely
- Documenting responses to treatments and changes in physical and emotional health to the case manager
- Ensuring seamless care by collaborating with nursing staff and other healthcare professionals to uphold nursing standards in home environments
- Providing education, guidance, and support to patients and their families regarding care plans
You will report directly to the RN Team Leader and the Director of Nursing.
A valid RN license is essential. Experience in home health is advantageous but not mandatory.
We seek individuals with a positive demeanor, strong nursing capabilities, effective communication skills, and a proactive approach.
This role offers flexibility, allowing you to manage your own schedule without the requirement to work multiple shifts. We provide a competitive salary commensurate with experience.
