Your responsibilities will encompass:
- Evaluating patients' health status and nursing requirements
- Establishing health objectives for patients
- Monitoring and analyzing patient symptoms
- Communicating treatment responses and any changes in the patient's physical and emotional state to the case manager
- Ensuring seamless care by collaborating with nursing staff and other healthcare professionals to uphold high standards of nursing practice in a home environment
- Providing education, guidance, and support to patients and their families regarding the care plan
You will report directly to the RN Team Leader and the Director of Nursing.
A valid RN license is mandatory. While experience in home health is preferred, it is not a requirement.
We seek individuals with a positive demeanor, strong nursing capabilities, effective communication skills, and a proactive approach.
This position allows for flexible scheduling, and you will not be required to work multiple shifts. We offer a competitive salary commensurate with experience.
