Empowering Veterans Through Supportive Services

The role of the Housing Case Manager within the Veterans Administration is pivotal in assisting individuals enrolled in the VA Grant Per Diem (GPD) housing initiative. This position entails conducting thorough evaluations, formulating personalized service plans, and tracking client progress to ensure housing stability and self-sufficiency.

Key Responsibilities:

  • Deliver weekly case management services both in clients' residences and within the community.
  • Facilitate access to essential resources and services for Veterans.
  • Collaborate with the VA GPD Liaison and participate in regular client meetings to assess and enhance service delivery.
  • Provide mental health rehabilitation services aimed at reducing hospital admissions and enhancing community integration.
  • Ensure continuity of care while adhering to ethical, legal, and regulatory standards.

Qualifications:

  • Bachelor's Degree with a minimum of two years of experience in mental health services.
  • Proven background in mental health and/or co-occurring disorders.
  • Strong written and verbal communication skills for accurate documentation.
  • Exceptional organizational abilities and proficiency in computer applications.
  • Ability to work autonomously and collaboratively within a team.
  • Experience in individual, family, and group counseling.
  • Valid driver's license and dependable transportation.

Work Schedule:

This is a full-time position with a standard schedule of Monday to Friday, 8:30 AM to 5 PM, though flexibility may be required.
