ROLE SIGNIFICANCE:

The Community Network Coordinator (CNC) is vital to Upward Health's mission of delivering healthcare directly to patients' homes. This role involves identifying and linking patients with Community-Based Organizations (CBOs), which are typically nonprofit entities that offer educational resources, vocational training, and various support services tailored to meet the unique needs of community members. By fostering connections with these organizations, the CNC helps patients address social determinants that influence their overall health. The CNC will cultivate and sustain collaborative relationships with local CBOs to guarantee that essential services are accessible to patients throughout their health journey. Additionally, the CNC advocates for patients by translating their needs into actionable plans for our care teams, ensuring that support gaps are effectively addressed. The ideal candidate will reside within the community served and possess a profound understanding of the available local resources.

PRIMARY DUTIES:

  • Develop and maintain a network of CBOs that cater to the diverse needs of Upward Health patients, regularly updating the list as services evolve.
  • Act as the main liaison for community partners functioning as CBOs.
  • Facilitate a cohesive network of clinical and community partners to enhance patient care coordination when CBOs are integrated into care plans.
  • Collaborate with clinical and non-clinical staff to assess patient health and needs, effectively connecting them with appropriate CBO resources.
  • Represent the community in which our patients live.
  • Provide guidance to Providers and non-providers regarding local CBOs, including available programs, service hours, and contact information.
  • Perform additional tasks as necessary to support organizational goals.

SKILLS AND QUALIFICATIONS:

  • Strong interpersonal skills, with the ability to engage and influence diverse individuals.
  • Experience in coordinating efforts across teams and external partners to fulfill the needs of patients or community members.
  • Background in social services.
  • Proficient in engaging with individuals from various cultural backgrounds, adjusting communication style as required.
  • Exceptional written, verbal, and digital communication skills, with a proactive approach to team interactions.
  • Effective listening abilities, quick decision-making skills, and a compassionate approach to addressing concerns.
  • Excellent organizational and presentation capabilities.
  • Ability to manage multiple schedules and adapt to the dynamic demands of various stakeholders.

EDUCATIONAL AND PROFESSIONAL REQUIREMENTS:

  • Residence within the patient service area is essential.
  • Possession of a valid driver's license and access to a vehicle with current insurance.
  • Extensive experience working with or within Community-Based Organizations.
  • Flexibility to work varied hours, including occasional evenings and weekends.
  • A bachelor's degree is preferred.
  • Willingness to travel within the patient service area.
  • Strong verbal and written communication skills, with a talent for building trusting relationships.
