ROLE SIGNIFICANCE:

The Community Network Coordinator (CNC) is vital to Upward Health's mission of delivering healthcare directly to patients' homes. This role focuses on identifying and connecting patients with Community-Based Organizations (CBOs), which are typically nonprofit entities that offer educational resources, vocational training, and various support services aimed at assisting community members. By fostering relationships with these organizations, the CNC helps patients tackle social determinants that influence their health outcomes. The CNC advocates for patients by articulating their needs and collaborating with our clinical teams to bridge any gaps in support. Ideally, the CNC resides within the community they serve, possessing a profound understanding of local resources available to patients. This position is grounded in our core values of safety, integrity, quality service, and a steadfast commitment to patient outcomes.

PRIMARY DUTIES:

  • Develop and maintain a network of CBOs that cater to the unique needs of Upward Health patients, ensuring the list is current and comprehensive.
  • Act as the main liaison for community partners operating as CBOs.
  • Facilitate a cohesive network of clinical and community partners to enhance patient care coordination when utilizing CBO resources.
  • Collaborate with both clinical and non-clinical staff to assess patient health and needs, connecting them with appropriate CBO services.
  • Represent the community perspective in patient care discussions.
  • Provide guidance to providers and non-providers regarding local CBO programs, including service offerings and contact information.
  • Perform additional responsibilities as required to support organizational goals.

SKILLS AND QUALIFICATIONS:

  • Strong interpersonal skills, with the ability to engage and influence diverse individuals.
  • Experience in coordinating efforts across teams and external partners to meet the needs of patients or community members.
  • Background in social services is preferred.
  • Cultural competency and adaptability in communication styles to connect with individuals from various backgrounds.
  • Exceptional written and verbal communication skills, with a proactive approach to team collaboration.
  • Active listening abilities, quick problem-solving skills, and a compassionate demeanor.
  • Excellent organizational and presentation capabilities.
  • Proficient in managing multiple schedules and responding to the dynamic demands of various stakeholders.

EDUCATIONAL AND PROFESSIONAL REQUIREMENTS:

  • Residence within the patient service area is mandatory.
  • Possession of a valid driver's license and access to a vehicle with current insurance.
  • Extensive experience working with Community-Based Organizations is essential.
  • Flexibility to work varied hours, including nights and weekends as needed.
  • A Bachelor's degree is preferred.
  • Willingness to travel within the designated patient service area.
  • Strong verbal and written communication skills, with an emphasis on building trusting relationships.
