Key Responsibilities of a Palliative Care LPN:
You will report directly to the Administrator or Executive Director.
Deliver direct patient care services to hospice patients and provide supportive care to their families while managing a reasonable caseload.
Assess patient and family needs and assist in the development and implementation of individualized care plans for each patient.Communicate findings or changes in the patient's condition to the Registered Nurse, interdisciplinary team, and referring physician, and document all observations.
Participate in staff meetings.Qualifications:
Essential Attributes:

Comfortable providing care in various settings as designated by the patient.
Exhibit flexibility and composure while supporting others during challenging times.
Licensed Practical Nurse / LPN license valid in the state of employment.
1+ years of experience in patient care within hospice, home health, ICU, geriatric, or oncology settings is strongly preferred.
Additional preferred experience that may enhance your success in a hospice role includes: hospital Staff LPN, Staff LVN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, nursing home, or travel nursing.
Possess a current driver's license and automobile insurance.Hold a current CPR certification.
