Role: Chronic Care Management Nurse (RN or LPN)

Reporting To: Chief Quality Officer

Key Responsibilities:

The Chronic Care Management Nurse plays a vital role in enhancing Population Health Management by:

  • Conducting weekly patient outreach to review medications and establish patient objectives.
  • Managing patient scheduling, maintaining patient lists, and overseeing billing processes for consultations.
  • Supporting Quality Assurance initiatives while engaging with patients.

Additional responsibilities include:

  • Performing triage for all patients prior to physician visits, which includes measuring weight, vital signs, and collecting chief complaints, while ensuring all necessary intake forms are completed.
  • Administering injections as prescribed by the physician or nurse practitioner.
  • Preparing examination rooms and necessary supplies for various medical procedures, assisting healthcare providers as required in executing these procedures, including gynecological exams, joint injections, minor surgeries, and suturing.
  • Conducting x-rays as directed by the physician or nurse practitioner.
  • Assisting front desk staff with phone inquiries, prescription refills, and appointment scheduling as needed.
  • Maintaining inventory of medical equipment and supplies in each examination room.
  • Facilitating referral appointments and coordinating outpatient imaging as ordered by the physician or nurse practitioner.
  • Helping with patient documentation, including forms for patient assistance, diabetic supplies, durable medical goods, and FMLA paperwork.

Qualifications:

  • Associate degree as a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
  • Three to five years of experience in an outpatient clinical environment.
  • Current BLS certification.
