Patient Navigator

Cahaba Medical Care Foundation is seeking a highly skilled professional to join our team as a Patient Navigator/Data Analyst. This exciting role will play a critical part in our clinical quality assurance/quality improvement program.
About the Role
The successful candidate will be responsible for managing data for our clinical quality assurance/quality improvement program, working closely with our CQO to achieve improvement in QA/QI measures. This will involve collecting and reviewing data, performing quality assessments, and collaborating with staff across all office locations to facilitate data collection and entry.
Key Responsibilities
- Manage data for our clinical quality assurance/quality improvement program
- Collect and review data, perform quality assessments, and follow up as necessary
- Collaborate with staff to facilitate data collection and entry
- Perform patient follow-up by phone call to schedule appointments and gather or update information
- Help manage the diabetic population in receiving their eye exam and provide assistance as needed
- Train staff in sliding fee processing and transportation assistance
- Manage optos and digital diagnostic schedules for Jefferson County clinics
- Report end of month data as set by CQO
Requirements
- Familiarity with Athena electronic medical record is strongly preferred
- Proficiency with Microsoft Excel and Google Apps
- Excellent attention to detail
- Excellent phone etiquette and customer service skills
