Job Opportunity

Cahaba Medical Care Foundation is seeking a highly skilled professional to join our team as a Patient Navigator/Data Analyst. This exciting role will play a critical part in our clinical quality assurance/quality improvement program.

About the Role

The successful candidate will be responsible for managing data for our clinical quality assurance/quality improvement program, working closely with our CQO to achieve improvement in QA/QI measures. This will involve collecting and reviewing data, performing quality assessments, and collaborating with staff across all office locations to facilitate data collection and entry.

Key Responsibilities

  • Manage data for our clinical quality assurance/quality improvement program
  • Collect and review data, perform quality assessments, and follow up as necessary
  • Collaborate with staff to facilitate data collection and entry
  • Perform patient follow-up by phone call to schedule appointments and gather or update information
  • Help manage the diabetic population in receiving their eye exam and provide assistance as needed
  • Train staff in sliding fee processing and transportation assistance
  • Manage optos and digital diagnostic schedules for Jefferson County clinics
  • Report end of month data as set by CQO

Requirements

  • Familiarity with Athena electronic medical record is strongly preferred
  • Proficiency with Microsoft Excel and Google Apps
  • Excellent attention to detail
  • Excellent phone etiquette and customer service skills
