Position Overview

Sign-On Bonus: This role includes a $3,000 sign-on bonus.

Work Schedule: Full-time | 80 hours per pay period | Monday-Friday | 8 am – 4:30 pm

Role Responsibilities:

  • Deliver personal assistance to clients in their residences under the guidance of a healthcare professional.
  • Perform light housekeeping tasks to maintain a supportive home environment.
  • Enhance the overall well-being of clients, addressing their physical, emotional, and social needs.

Key Duties:

  • Assist with personal care activities as instructed by the supervising nurse or therapist, including bathing, wound care, dressing, transferring, and toileting.
  • Foster positive relationships with clients and their families to support social well-being.
  • Conduct light housekeeping duties as directed by the client to ensure a clean and safe living space.
  • Maintain accurate documentation of client care in accordance with established policies and communicate updates to the supervising healthcare professional.
  • Responsible for setting up personal emergency response systems (PERS) for clients.
  • The responsibilities outlined above are representative of the nature and level of work expected and are not exhaustive.

Qualifications:

  • Must be a registered CNA in the State of Minnesota.
  • CPR certification is required within six months of hire.
  • A valid driver's license and access to an insured personal vehicle are necessary.
  • Completion and passing of a state-required background study are mandatory.
  • Effective verbal and written communication skills are essential for the role.
  • Ability to physically perform the tasks required by the position is necessary.
  • Proficiency in reading, writing, and speaking English is required to effectively communicate with staff, clients, and visitors.

Reporting Structure: This position reports to the Regional Clinical Manager.
