Job Summary

UCFS is seeking a highly skilled and compassionate Registered Nurse to join our Solution Center team. As a key member of our team, you will provide exceptional patient care by assessing and prioritizing patient needs over the phone. Your expertise will ensure patients receive the appropriate level of care.

Key Responsibilities
  • Respond promptly to incoming calls and provide nursing triage as calls warrant.
  • Redirect patients who do not need emergency care to their primary care provider.
  • Provide education on home care for minor illnesses and make recommendations for further care.
  • Collaborate with physicians and other healthcare professionals to ensure patients receive the best possible care.
  • Oversee the Chronic Care Management program and direct calls and messages to the appropriate care teams.
Requirements
  • Graduate of an accredited School of Nursing, BSN preferred.
  • One to three years of experience, preferred Ambulatory Care experience.
  • Active Connecticut Registered Nurse license.
  • Strong critical thinking and problem-solving skills.
  • Excellent communication & interpersonal skills to place clients/patients at ease.
