As a Medical Care Specialist, you will play a vital role in our interdisciplinary team, working closely with patients, families, and healthcare professionals to develop and implement personalized care plans.

Key Responsibilities:

  • Conduct comprehensive assessments of patients' medical and social needs
  • Develop and update care plans to ensure optimal health outcomes
  • Collaborate with patients, families, and healthcare professionals to achieve care goals
  • Prescribe diagnostics and interventions to prevent unnecessary hospitalizations
  • Provide education and support to patients and families on disease management and wellness
  • Participate in quality improvement initiatives to enhance care delivery and patient satisfaction

Requirements:

  • Completion of a Nurse Practitioner or Physician Assistant program
  • Current state RN/NP or PA license
  • Certification through a recognized professional organization
  • Access to reliable transportation

Preferred Qualifications:

  • 2+ years of clinical experience in primary care, home-based medical care, or geriatrics
  • Knowledge of long-term care, community resources, and cost-effective alternatives
  • Knowledge of CPT, ICD-10, and HCPC codes
