As a member of our Landmark product team, you'll play a vital role in bringing home-based medical care to complex, chronic patients, helping them access and navigate care anytime and anywhere.

We're committed to improving the health and well-being of our patients, and we're seeking a skilled Medical Care Specialist to join our interdisciplinary team. In this role, you'll work closely with patients, primary care physicians, and other healthcare professionals to develop and implement personalized care plans that address the unique needs of each patient.

Key Responsibilities:

  • Conduct comprehensive assessments of newly enrolled patients and provide ongoing care and support
  • Develop and update patient care plans in collaboration with the patient's primary care physician and other healthcare professionals
  • Prescribe diagnostics and interventions to prevent unnecessary acute admissions
  • Provide education and support to patients and their families on disease management and wellness strategies
  • Participate in ongoing quality improvement initiatives to ensure the highest standards of care

Requirements:

  • Completion of a Nurse Practitioner or Physician Assistant program
  • Current state RN/NP or PA license
  • Certification through a recognized professional organization
  • Access to reliable transportation

Preferred Qualifications:

  • 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
  • Knowledge of long-term care, community resources, and cost-effective alternatives
  • Knowledge of CPT, ICD-10, and HCPC codes
