Position Overview:
The Substance Use Recovery Specialist plays a crucial role within the innovative Hub and Spoke Health Home model, dedicated to supporting individuals facing significant challenges related to substance use and co-occurring mental health issues. This position involves close collaboration with the Hub and Spoke Team to create tailored care plans, conduct regular recovery assessments, and ensure ongoing support for individuals in their recovery journey.

Key Responsibilities:

  1. Engage effectively with individuals using a strength-based, trauma-informed, and culturally sensitive approach, emphasizing the importance of participation in care and services.
  2. Demonstrate a strong commitment to assisting individuals with substance use and mental health needs.
  3. Work in partnership with the Lead Recovery Navigator and the Hub Team, including Certified Peer Specialists and Wellness Coaches, to provide comprehensive support to program participants.
  4. Deliver direct services to approximately 12 individuals each week, focusing on their unique needs and goals.
  5. Conduct a Recovery Life Story Tool and Assessment (RLST/PRP) in the individual’s own words, emphasizing their strengths and strategies for achieving their objectives, with updates every 90 days.
  6. Ensure comprehensive care addressing various needs, including medical, housing, legal, financial, and social aspects.
  7. Facilitate monthly monitoring and team meetings in collaboration with the individual’s care team.
  8. Coordinate with service providers to verify that necessary services are delivered, maintaining regular communication with care team members.
  9. Assist individuals in building a network of informal and formal support systems based on their preferences.
  10. Provide transportation and accompany individuals to appointments, advocating for their identified needs.
  11. Support crisis intervention and develop immediate response strategies, including comprehensive relapse prevention plans.
  12. Adhere to all organizational policies and procedures, fostering a positive and supportive culture.
  13. Document all interactions and activities related to individual care in the electronic medical record promptly.
  14. Participate in required training and uphold confidentiality in all communications.

Qualifications:

  • High school diploma or GED required; a Bachelor’s degree in Social Work, Psychology, Nursing, or a related field is preferred.
  • Experience working with individuals facing substance use challenges.
  • Proficiency in computer applications, particularly Microsoft Office.
  • Ability to demonstrate cultural competence and respect for diverse backgrounds.
  • Valid driver’s license and reliable transportation.

Experience in supporting individuals with substance use and mental health needs, knowledge of evidence-based practices, and strong communication skills are essential. The ability to work independently while being a collaborative team player is crucial.

Working Environment:
This role involves both office and community settings, requiring travel to various locations to support individuals enrolled in the program. Flexibility in scheduling is necessary to meet the needs of both staff and program participants.
